If you have insurance through a company with which we have contracted, we will bill your insurance company for your visit. We will require a copy of your insurance card and driver’s license. All Co-payments, co-insurances and deductibles are due at the time of your visit. You will be responsible for any services denied by your insurance carrier as not medically necessary and/or not covered. Cosmetic procedures are not billed through insurance and are payable in-full at the time of service. We accept Visa, Mastercard and American Express.
Please keep in mind that it is your responsibility to know the benefits provided by your insurance company. It is not our responsibility to know the nuances of each insurance plan. Please call them in advance if you are uncertain of your coverage.
